Home Health Care Inc.
is seeking Physical Therapists throughout the Twin Cities Metro area! The Physical Therapist will be responsible for providing physical therapy services to clients in their homes.
The Physical Therapist will develop, implement, and evaluate treatment plans and provide education to clients, family members, and other healthcare professionals in order to maximize the client’s independence and quality of life.

Physical Therapist / DPT Qualifications:
* Possess a current PT / Physical Therapy license to practice in the State of Minnesota
* Experience in home care setting, namely OASIS documentation
* Valid Driver's License and reliable transportation
* Excellent communication and interpersonal skills.
* Ability to work independently and as part of a team.
* Proficiency in Microsoft Office programs.
* Ability to travel to patient homes in the local area.
Physical Therapist / DPT Overview:
* Evaluate and assess a patient's physical condition and functional capabilities in order to develop an individualized plan of care.
* Provide physical therapy treatment and interventions to clients in the home setting.
* Educate and instruct clients, family members, and other healthcare professionals on therapeutic exercises and activities.
* Develop and implement an individualized plan of care for each patient.
* Monitor and document patient progress in accordance with regulatory standards.
* Collaborate with other healthcare professionals in order to provide comprehensive care.
* Adhere to ethical and professional standards and comply with all regulations.
